Appointments

To schedule appointments, please call our office during business hours. We realize that your time is important to you as we attempt to stay on schedule with your appointment. Occasionally, however, unavoidable delays occur. Your provider may be in surgery, or perhaps called to an emergency. We will keep you informed if a delay arises.

If you cannot keep your appointment, please notify us as soon as possible--preferably 24 hours in advance. Our office will charge a $35.00 fee for missed appointments which your insurance will not cover.

Please have your copay at the time of your visit. There is a $10 service fee if copay is not paid at time of service. We appreciate your cooperation.

When you arrive, please be sure to have your Patient Registration Form ready.
