Welcome. If you are seeking existing patient information, you've come to the right place. Below are several forms, which should be reviewed/filled out if necessary and either mailed/faxed back to our office. Other general information is offered below as well. Other General Information... Refilling Prescriptions All prescription refills should be requested by calling your pharmacy. The pharmacy will then contact our office. Do not call our office before calling the pharmacy. Please allow approximately three (3) days for processing. Missing Appointments If you cannot keep your appointment, please notify us as soon as possible--preferably 24 hours in advance. Our office will charge a $35.00 fee for missed appointments which your inc\surance will not cover. We appreciate your cooperation. X-Rays & MRI Patients are repsonsible to hand-carry all relevant radiological studies personally. These studies or films must be picked up from the facility at which they were conducted. Please make arangements well in advance of your scheduling appointment. Pre-Operation Patients Pre-Operational patients must follow the Doctors orders to ensure the most positive surgical environment and post-surgery effects. At the top of the page you will find the Pre-Op form given by the medical assistant of your physician in aide to your preparation for your surgery. Please click the link ot help inform you further.
